I use the free service at http://polldaddy.com to create an applicant survey (limited to 10 questions with the free version)
it really helps to,
- screen applicants better to save time in the interview process
- begin outlining expectations by listing minimal requirements
- keep the interview process professional without being too formal
- can be posted on ad pages such as craigslist or sent by email
- meet the changing climate of online applicants
we cover,
- age eligibility for hiring needs
- outlining requirements for working weekends and holidays
- determining availability
- guest service experience
- ability to meet presentation guidelines
- cash handling experience

p.s. I have started to use an interview schedule document thru Google docs to have applicants pick their own interview time from a list of structured times
